To be eligible for admission to Grace Plaza Nursing & Rehabilitation Center, an individual must require 24-hour skilled nursing care, rehabilitation/sub-acute treatment, hospice care or respite. Our admissions staff will gladly help you and answer any questions you might have about your loved one’s eligibility for admission to Grace Plaza.

Will my insurance cover my stay?
Medicare, Medicaid and Managed Care Insurance cover an eligible patient’s stay for a defined period of time. Private payment is accepted for those eligible patients who require a stay but are not covered by insurance.
What are the steps for admission to a long or short term care facility?
Prior to an applicant’s consideration for admission, a Patient Review Instrument (PRI) must be completed by a certified registered nurse. These forms are reviewed by our clinical team to determine eligibility for care. Upon availability of an appropriate bed, the eligible patient is admitted.

• A hospital – the hospital discharge planner can fax the PRI and Screen to our Admissions office for evaluation at 516-829-3854.
• Home – a PRI and Screen may be completed by a home care nurse or a visiting nurse and faxed to our Admissions Office at 516-829-3854.
• Another facility – you may request that the social work department fax PRI and Screen to our Admissions Office at 516-829-3854.
