Accredited by the Joint Commission on Accreditation of Healthcare Organizations Since 1975

Grace Plaza Nursing and Rehabilitation Center was established in 1972. Located in a five story building in Western Nassau, Grace Plaza has been consistently awarded accreditation by the JCAHO. It is conveniently located and is easy to get to from major roads and highways, the Long Island Railroad, and area hospitals (for specific directions, please see the Directions To Our Facility).

Visiting hours: 11:00 AM to 9:30 PM daily

Valet Parking: A parking attendant is on duty from 8:30 AM to 4:30 PM, Monday through Friday. His job is to make parking as easy as handing him your keys. After 4:30 PM, keys can be retrieved at the Reception Desk.

Services: Grace Plaza offers progressive, quality healthcare in an atmosphere that speaks to the unique needs and challenges of every individual in our care. We have learned to understand and respond to the complicated needs of the families we serve. Our programs and services are designed to provide education, support and counsel in every aspect of care.

Grace Plaza provides a variety of healthcare services. Our Pathways unit is designed for short-term patients who are in transition from hospital to home. We also provide traditional long-term care, and specialized services for persons with Alzheimer's disease. Whatever your unique circumstances, we are confident that we can meet your needs.
