Recently Grace Plaza celebrated their staff during Therapeutic Recreation Week. Our Recreation Department is tremendously dedicated to our residents by organizing events and creating activities in their daily lives. Their hard work and dedication is very appreciated and shows as smiles on our residents faces everyday. Great job!
 

Chief Registered Dietician Visits Great Neck Senior Center

Sharon Patel, Chief Registered Dietician at Grace Plaza Nursing & Rehabilitation Center gave an educational lecture to the guests of Great Neck Senior Center.  Sharon’s lesson was focused on “Tips for Healthy Eating”.  The guests were given simple easy ways to incorporate healthy eating into their everyday life.   The goal was to educate the seniors in the community while also offering them options.  This is one way in which Grace Plaza is committed to the community by offering education and information. 
 

Neurological Coma Recovery Program

Grace Plaza has developed an expanded Neurological Coma Recovery Program, in consultation with Dr. Elizabeth Wirkowski, who are not vent dependent, in various stages of coma recovery. All patients receive an individualized treatment plan designed specifically to their needs with the collaboration of their physician and a multidisciplinary team.

The goal of the Coma Recovery Program is to regain the highest and most independent level possible with continuous assessment conducted by the team to trace the progress in this process of recovery.

Caring for someone who has suffered neurological damage or is currently in a coma can be a daunting task. The program is for individuals and families ready to embark on a unique therapeutic approach to coma recovery through a multidisciplinary program. Resident, Syde Swern, Celebrates 103rd Birthday!

On December 25, Syde Zwern, a 4-year resident at Grace Plaza celebrated her 103rd birthday. Syde was born on December 25, 1912 in Queens, New York to her parents Jacob and Celia Olch. She met her husband Louis and they were married in 1942. Syde and Louis raised two daughters Jacqueline and Reina in their home in Rosedale, Queens. Louis passed away in 1986 after 44 years of marriage. When asked for advice on how she stays so positive and young, Syde replied, “I don’t think about my age, I just do what I have to do and try to be thankful for all I have.” Today Syde is blessed to have 6 grandchildren and 6 great-grandchildren. Syde is an avid reader of the New York Times and stays busy by attending the activities, enjoying the entertainment and her friends at Grace Plaza. She participates in the weekly discussion groups and assists in the Doer’s Club which raises money for outside charities as well as other residents in need. A party, attended by her family, was held at Grace Place to celebrate this special day and we feel we are very blessed & honored to be able to share this day with her.
 

Clasp Day Care Center Visits Grace Plaza

The residents of Grace Plaza and the children of Clasp Day Care Center recently held their annual holiday celebration. The children presented their adopted grandmothers and grandfathers with beautiful centerpieces they made for the holiday tables. The children and residents shared snacks and conversation and the children delighted the residents with songs. Each child then received a special present from the residents.

Clasp Day Care Center and Grace Plaza have shared this special relationship for over 20 years. The children, who range in age from 3-4, make weekly visits to their friends at Grace Plaza. It’s a rewarding program for all and gives everyone a weekly treat. The staff of both organizations work together to make it a meaningful and rewarding experience and look forward to another 20 years of sharing!

Grace Plaza Welcomes New Director of Food & Nutritional Services

Grace Plaza Nursing & Rehabilitation Center is proud to welcome Keisha Johnson, as the new Director of Food & Nutritional Services.

Keisha comes to Grace Plaza with an extensive background and experience in Food Services. Prior to her employment at Grace Plaza, Keisha worked as Dietary Supervisor at Avalon Gardens in Smithtown as well as the Restaurant Manager for Denny’s in Centereach.  Keisha will oversee the daily operations of the Food & Nutritional Services Department at Grace Plaza and her extensive experience and professional background will be a great asset for the staff, families and residents. Keisha resides in Wheatley Heights with her son Christian.

Grace Plaza Welcomes New Director of Admissions

Grace Plaza Nursing & Rehabilitation Center is proud to welcome Debra Fenner, as the new Director of Admissions.

Debra comes to Grace Plaza with an extensive background and experience in Admissions. Prior to her employment at Grace Plaza, Debra worked for over 20 years as Admissions Coordinator at Nassau Extended Care in Hempstead.  Debra will oversee the daily operations of the Admissions Department at Grace Plaza and her extensive experience and professional background will be a great asset for the staff, families and residents. Debra resides in Uniondale with her husband and three children.

Grace Plaza Welcomes New Administrator

Grace Plaza Nursing & Rehabilitation Center is proud to announce the appointment of Kim Sodano, LNHA, LCSW, as Administrator. Prior to her employment at Grace Plaza, Kim was the Administrator of Brookhaven Rehabilitation and Health Care Center. Kim received her Master’s Degree in Social Work from Adelphi University and comes with years of experience as a Clinical Social Worker working within her community and is excited to oversee the daily operations of Grace Plaza with her focus and goals to improve the quality of care and quality of life for the residents.

Grace Plaza Residents Visit Red Lobster

The staff of the Recreational Therapy Department at Grace Plaza recently treated the residents to a special luncheon at Red Lobster. The residents enjoyed being able to share the day with each other, being out to enjoy the weather and dining on a special meal. It’s special events like this that our staff strives to bring to the residents to enhance their lives and make Grace Plaza the special place it is.
 

Nursing Home Week 2015

On Wednesday, May 13, 2015, Grace Plaza celebrated their amazing staff during Nursing Home Week.  The staff were treated to a delicious lunch outside on the patio as well as given an opportunity to win some great raffle prizes.  At Grace Plaza we appreciate the dedication and professionalism of our staff, and this was our way of saying thank you for all they do.
 

Grace Plaza Introduces Advanced Program for the Treatment of Swallowing Disorders

Grace Plaza has recently installed a leading-edge system for the treatment of swallowing disorders, commonly known as “Dysphagia”. This unique system, ACP Synchrony, is the first to use visual-reality feedback, which allows the Speech Language Pathologist to more effectively guide treatment efforts and monitor patient progress. For residents, this allows for a fun and interesting version of therapy. This new treatment used at Grace Plaza will give us the ability to treat our residents more effectively and quickly.
 

Grace Plaza Celebrates the Persian New Year of Norooz

On March 23, 2015, Grace Plaza celebrated the Persian New Year of Norooz.  Residents were able to share a traditional luncheon with their families.  Norooz is an ancient holiday celebrating renewal and rebirth symbolized by the coming of spring. At Grace Plaza we are happy to be able to bring this tradition to our residents and offer them a day to share this holiday with their families.

Volunteer Opportunities at Grace Plaza

 

Grace Plaza Nursing & Rehabilitation Center is currently seeking compassionate individuals willing to volunteer their time.  There are many opportunities available to volunteer, such as assisting with resident recreational programs, transporting or visiting with patients. Interested individuals must be at least 13 years of age.

Grace Plaza Welcomes New Director of Social Services

Grace Plaza is proud to welcome Mary Ellen Swersky as the new Director of Social Services. Mary Ellen comes to Grace Plaza with an extensive background and experience in social work receiving a Masters of Public Administration Degree from CW Post in Old Brookville. Prior to her employment at Grace Plaza, Mary Ellen worked as Director of Social Work at Manhattanville Health Care Center in the Bronx. Mary Ellen will oversee the daily operations of the Social Work Department and her extensive experience and professional background will be a great asset for the staff and residents.

Grace Plaza is proud to welcome Francesca Welhous, MT-BC, as the new Therapeutic Recreation Assistant. Francesca comes to Grace Plaza with an extensive background and experience in music therapy receiving a Bachelor of Science Degree from Molloy College. Francesca will assist the Therapeutic Recreation Department with the daily programs and activities and her musical talent and background will bring much enjoyment to the residents

On Monday, December 22, 2014, The Giving Tree Family organization visited Grace Plaza.  For the third year in a row, these wonderful talented individuals entertained residents with their songs and musical talents. The parents and children also generously donated holiday gifts to each resident. Seeing the delight and smiles on their faces are what the joy of the holidays are all about.
