When Grace Plaza Nursing and Rehabilitation Center opened in 1972, its goal was to continue
the pattern of care and caring established by loving families when it was no longer possible
to do so at home. Today, the skilled and dedicated staff of Grace Plaza continues in that
tradition, offering sophisticated medical, nursing and therapeutic care in a warm homelike
environment. Grace Plaza employs a professional staff whose education and experience enables
them to provide the highest quality of care to our residents.
The 214-bed facility is graciously appointed with rooms having one or two beds, private bathroom,
air conditioner and electronic call bell system. A beautifully landscaped patio and balconies on
each floor provide an outdoor setting for residents and their visitors to gather. There are specially
equipped rooms for supervised activity, physical and occupational therapy and beauty parlor and
barber services. Each dining lounge is equipped with a large screen color television and a VCR so
that taped family events may be shared and popular movies are viewed.
Recently a three million dollar renovation has been completed in all patient care areas. All aspects
of the facility have been updated including new furniture in PT rooms and dining rooms and all
bathrooms have been retiled.
In Our Home We Care
A multi-disciplinary team composed of a physician, nurse, dietician, social worker, rehabilitation
therapist and recreation specialist periodically review each resident's plan of care to ensure that
current medical, social and emotional needs are being fully met. Dentistry, lab, x-ray services, all
therapies and medical specialists are available to residents at the facility.
Well-balanced and delicious meals are carefully planned by registered dieticians. Individual dietary
requirements and personal preferences are always considered. Meals are served in an atmosphere designed
to promote socialization and relaxation. Menu alternates are always available and nourishments are served
in the afternoon and evening. Holiday celebrations include a variety of ethnic foods. Frequent parties
and planned events give residents the opportunity to enjoy the special treats which nourish the spirit
as well as the body.
