At Grace Plaza our nursing staff has the knowledge and compassion to serve the disabled, and those recuperating from illness or injury in a caring environment. The Charge Nurse reviews our plan of care with the attending physicians and works in conjunction with the clinical team, including your Certified Nursing Assistants, to assure you the best possible care. Information regarding your care is discussed with all involved caregivers on every shift. An Administrative Nurse Supervisor is on duty 24 hours a day. If you have any questions or concerns that cannot be resolved by your Charge Nurse, please ask a Supervisor. Nursing care is provided through an interdisciplinary approach to maximize the highest quality of care received by our residents. Specialized programs are provided to enhance positive clinical outcomes as well as enriching the patient’s/resident’s quality of life. These goals are achieved by involving family members and working together to facilitate desirable outcomes.
